Demonstration content. I waited nearly a year after diagnosis before telling my manager, mostly out of worry about how it would be received.
In the end I asked for a meeting with HR present rather than just my line manager, and I brought a one-page summary of the adjustments that help me, such as written instructions instead of verbal ones.
Everyone's workplace and comfort level is different. This describes my own experience, not a recommendation for what you should do.
